Disease Management to Population-Based Health: Steps in the Right Direction?

This meeting examined the evolution of the disease management model and the varied approaches taken by different organizations to implementing disease management for their beneficiary populations. It looked at the experience of population-based disease management programs operating in both the private and public sectors and reviewed the evidence of their success. It also explored the policy implications of adapting this model to a Medicare fee-for-service population.
